GLP-1 coverage by state
Only 13 state Medicaid programmes cover GLP-1s for obesity. Four dropped coverage in the last year. Here is the national picture and where it changed.

States where Medicaid does cover GLP-1s for obesity
Michigan, Tennessee, Virginia, Minnesota, Wisconsin, Missouri, Kansas, Delaware all cover GLP-1s for obesity treatment, alongside Mississippi and Utah. If you are in one of these states and on Medicaid, that is almost certainly a better route than anything you can buy for cash — enrollees generally pay little or no copay for prescription drugs.
Two caveats. Coverage is typically subject to prior authorisation, and at least one covering state — Michigan — has narrowed eligibility to morbid obesity rather than eliminating coverage, a move expected to save it around $240 million. Coverage existing is not the same as you qualifying.
States where coverage has been proposed for removal
Massachusetts and Rhode Island both had live proposals to end Medicaid coverage of GLP-1s for weight loss as of April 2026. Neither had taken effect at the time of reporting. If you are eligible in either state, the sensible read is that the window may not stay open.
States that eliminated coverage
California, Pennsylvania, South Carolina, New Hampshire all ended coverage of GLP-1s for obesity after October 2025, which KFF attributes to budget pressure and drug cost.
States covering weight-loss drugs but no GLP-1s
Texas, Louisiana, Connecticut, New Mexico, North Dakota report covering one or more weight-loss drugs while covering no GLP-1 at all. It is the distinction that catches most people out, because a weight-loss benefit on paper reads like a yes.
Time-limited coverage
Utah reported that funding for coverage was limited to fiscal year 2026 — a deadline rather than a refusal.
The states at the extremes
West Virginia, Mississippi, Colorado stand out on prevalence rather than policy. Every US state and territory recorded adult obesity prevalence of at least 25% in 2024.

Medicare, which does not vary by state
