GLP-1 coverage in Texas
Medicaid covers weight-loss drugs but no GLP-1s. What that means, the indications that are still mandatory, and what it costs if you pay cash.

Where you live decides whether the state will help pay for this. Texas is one of the places where that answer recently changed.
Texas Medicaid covers weight-loss drugs but not GLP-1s.
KFF lists Texas among seven states that report covering one or more weight-loss drugs while covering no GLP-1s at all. That distinction catches people out: a weight-loss benefit in the plan documents does not mean semaglutide or tirzepatide is on the formulary. Everything below is sourced — we are not going to tell you what a Texas clinic charges, because every provider we track is 50-state telehealth and prices the same wherever you live. What actually differs by state is coverage, and that is what this page is about.
What a state decision actually changes
A coverage decision does not change the medicine, the dose or the evidence. It changes who pays, and therefore who can have it. That is worth sitting with: the same prescription is routine in one state and out of reach in the next, on the basis of a budget line rather than anything clinical.
Why Texas is different
Texas is the largest state on KFF's list of programmes covering a weight-loss drug while covering no GLP-1, which makes it the single biggest population affected by that particular distinction. It also sits in the South, where adult obesity prevalence runs at 34.5% regionally — so the gap between what the benefit appears to offer and what it delivers affects a lot of people.

Coverage for obesity is optional for states — the national picture is on our coverage index. Here is where Texas sits.

Paying cash in Texas
Providers are licensed across all 50 states and charge the same here as anywhere, so there is no local price to hunt for — the cheapest verified all-in rates are Ozari Health at $86 for semaglutide and Trimi at $125 for tirzepatide. Full tables in the cheapest tirzepatide. If you buy compounded, check the pharmacy licence with the Texas State Board of Pharmacy, or with the board in whichever state that pharmacy actually operates from — frequently not Texas.
Texas: common questions
Does Texas Medicaid cover Wegovy or Zepbound?
KFF lists Texas among the states covering one or more weight-loss drugs while covering no GLP-1s. Since Wegovy and Zepbound are both GLP-1s, a weight-loss benefit does not get you either of them for obesity.
Texas is huge. Does coverage vary within the state?
The fee-for-service position is statewide, but managed care plans can differ in how they administer utilisation controls. Ask your specific plan about the specific drug.
